The results speak volumes: a staggering 50% reduction in hospital re-admission rates among patients utilising the Total Cardio Care app. By fostering better medication adherence and facilitating rehabilitation exercises, we’ve not only enhanced patient outcomes but also alleviated the strain on hospital resources.
