Total Cardio Care: Revolutionising Transitional Care

One of our flagship initiatives, the Total Cardio Care app, stands as a beacon of innovation in transitional care for cardiac patients. Developed in collaboration with esteemed cardiologists Dr. Praveen Indraratna, Associate Professor Sze-Yuan Ooi, and Scientia Professor Nigel Lovell, this Australian-first smartphone app has revolutionised the transition from hospital to home. Through real-time telemonitoring and comprehensive education, the app empowers patients to take charge of their health journey with confidence.

The results speak volumes: a staggering 50% reduction in hospital re-admission rates among patients utilising the Total Cardio Care app. By fostering better medication adherence and facilitating rehabilitation exercises, we’ve not only enhanced patient outcomes but also alleviated the strain on hospital resources. Such success has prompted the expansion of the Total Cardio Care app to other hospitals, ensuring its life-changing benefits reach even more individuals in need.

 

Aortic Valve Replacement: Pioneering Minimally Invasive Solutions

Our commitment to pioneering advancements in cardiac care led us to fund the development of Transcatheter Aortic Valve Implantation (TAVI), a minimally invasive alternative to traditional open-heart surgery. This groundbreaking technique has not only streamlined the recovery process but has also significantly reduced the risks associated with invasive procedures.

TAVI represents a beacon of hope for patients deemed too high-risk for conventional surgery, offering a lifeline where previously there was none. By enabling quicker recovery times and minimising infection risks through smaller incisions, we’ve empowered patients to reclaim their lives with newfound vigour and vitality.

Recognising the pivotal role of advanced monitoring systems in ensuring patient safety and continuity of care, we proudly funded the implementation of state-of-the-art cardiac monitors at Prince of Wales Hospital.
